We wanted to share information about a FREE Community Health Clinic that will be offering a variety of health services and resources to families in our community.
Services will include:
Blood pressure screenings
Diabetes education
Dental screenings
Flu shots
Sports physicals
Vision exams
Community resources

Hours: Saturday: 8:00 AM-5:00 PM Sunday: 8:00 AM-2:00 PM
Eligibility Requirements:
Patient must be 5 years old or older. Household income must fall within the federal poverty guidelines.
Patient must not have medical insurance that covers a vision exam and/or glasses. Children may have Medicaid or MCHP. Patient or parent/guardian must provide consent to care by completing the required consent form.
Services are available by appointment, with limited walk-in availability. Families interested in participating should register using the QR code on the attached flyer or the link below.
